Yes,  you will have to report the dividends on your tax return.  You should have received  Form 1099-DIv or an equivalent statement reporting the dividends.

No, if the amount is less than $10 and you did not get a 1099-DIV. It is considered to be De Minimis.  Yes, if the amount is over $10 and you received a form 1099-DIV.

Yes, whether or not you received a statement from the financial institution reporting the income to you.  There is not a minimum income amount that must be met before you are required to include the income as part of your return.  In order to file a complete and accurate return, you are required to report all worldwide income.

 

The minimum dollar amount applies only to whether or not the financial institution is required to issue a Form 1099-DIV or Form 1099-INT.
