Distributions received from my employer’s bankruptcy.

My employer filed for chapter 7. I put in a claim for my retirement distribution. I was paid a partial amount of the total amount of the retirement. The bankruptcy judge stated the I will not be receiving any tax form from him.  Do I report this bankruptcy distribution? If so, is it earned income or 1099 income?
