Could someone review my input of my W2?

I spoke with a few co-workers that have a similar tax status that I do and they are getting much larger returns. I think that I may have input my W2 incorrectly, possible missing some of the 12 boxes.

If you think you may have filled out your return incorrectly, you may amend your return using the 1040X form. You may also want to check your withholding, allowances, dependents, and pretax contributions to FSA, MSA, and retirement plans.

Also, check which form your state requires. It will most likely have an "X" at the end. Most states also require a copy of your 1040X.
