Charged for Costco bought Premium Turbo Tax again on my Return

I was charged for a Premium TurboTax on my return when I had bought it at Costco. Does the program not have a way to determine when the version used is store bought? This has happened before but I never spoke up but I finally felt I must look into it. These charges are too large to ignore.

Issue number two, I was prompted to fill out the details for the Audit Defense Service even though I opted to not use the service.
