Can you deduct lost wages and do I need something showing the earnings still owed from the employer?

My former employer still owes me wages from a previous tax year.  Should I do an amended return to reflect these wages still owed and if yes, how and do I need a specific tax form and something from the employer?

No, you cannot deduct loss wages.  You never paid taxes on income you didn't receive.
