Yes, just include your best estimate for your jury duty pay. Since it is income, it should be included on your return, but there is not a tax document that reports that income to you or the IRS. If you find out later that you significantly underestimated the amount, you could always file an amended return to correct it.
unless you did a fill-up at the start and end, (and drove nowhere else) your fuel cost is going to be an approximation.
