State wouldn't accept my check without the required form.

Well here's the blank ones you can fill out since you know the amount. But the 4Th quarter is due tomorrow.

And....there is no mail tomorrow. It will not even go out on January 15. You will not be able to mail it until Tuesday. Maybe you should pay on the VA site.
