Box 16 on my 1099-R was empty; but, TurboTax would not send my return until I put the total state tax withheld in that box. Why is that?

If boxes 14 and 15 of your form 1099-R are populated and box 16 is empty, this means that the issuer of the form has omitted to fill it with the amount of distribution. You can safely enter the gross amount in box 1 in box 16.
