At the end, after I finished filing. There was a button to take a survey. I can't seem to get back to it, or find it again. Where is it?

Unfortunately, once you've moved on from the survey screen it will not be able to be viewed/completed.  If you would like to leave any feedback here, you are more than welcome to reply in this thread.

I mistakenly while taking the Federal Tax survey might of

click on DISAGREE STRONGLY  when I met to click on

AGREE STONGLY,  or something to that affect.  My time 

 with Rep. FOAD was extremely helpful and he provided  the highest customer service possible. We should have more customer service people like him.
