Are dividends paid from life insurance policy (amount paid out has exceeded cost of policy but no 1099-DIV is sent) considered "qualified"?

Are dividends paid from life insurance policy (amount paid out has exceeded cost of policy but no 1099-DIV is sent) considered "qualified"?
Unless the life insurance company has sent you a 1099-Div, you may not need to report these dividends.
You may want to contact the company if you feel you should have received a 1099-Div.
