I am at the final stage to amend the 2019 tax return. Thanks a lot for all the input that I have been receiving from the community. 4 additional questions:
1. The instruction from Turbotax says "attach all forms or schedules that changed to your amended return".
Since I am amending the 2019 tax return to report and pay penalty for Roth IRA excess contribution, the changes are only 1040X, 5329, and and a schedule 2. Shall I still include forms and schedules with no changes at all? Turbotax automatically prints out my original 1040, schedule B, and 8995. Since there is no changes on the original 1040 and schedule B, I am wondering if I should mail them to IRS as well.
2. For 1040X, am I supposed to fill out line 1-5 and 24-30 by myself, even if there is no changes to income&deductions and exemptions&dependents? Turbotax left them blank since I am not amending those items, but I could not find anywhere on the 1040X instruction that these lines can be left blank even if there is no changes.
3. According to https://www.irs.gov/forms-pubs/about-form-1040x, electronic filing is now available for Form 1040-X. I was wondering if Turbotax supports this, or I still have to mail the documents out to IRS.
4. Turbotax also printed out a page for electronic postmark - certification of electronic filing. This document certifies that the original 2019 tax return was filed back in 2020 February. Shall I also include this in the amended 2019 tax return?

1. No only include the forms that changed and the 1099-R.
2. No, leave form 1040X how TurboTax filled it out.
3. No, you will have to mail the 2019 1040X, TurboTax only supports to e-file the current tax 1040X.
4. No you do not need to include the page for electronic postmark.

1. Turbotax also printed out a schedule 2 "additional taxes" for my amended 2019 tax return. Yes I do own 28 dollars but it looks like the schedule 2 is for form 1040, not for form 1040X. Since there is no change for form 1040 (I am only amending the 2019 tax return to report and pay penalty for excess contribution into Roth IRA in 2019) and I am only filing 1040X, shall I still mail the schedule 2 as part of my amended 2019 tax return?
2. Now that I am not including the original 1040 (since there is no changes) but will include 1040X, shall I sign the form 5329? Instruction for 5329 says "sign only if you are filing this form by itself and not with tax return".

1. Please include new Schedule 2 since there should be the additional tax on line 6 from Form 5329. And the information from Schedule 2 should carry to the new Form 1040. Therefore, the changed Form 1040 has to be be included as well.
2. No, don't sign Form 5329 since you are filing 1040X.
Just to confirm.
To amend the 2019 tax return for Roth IRA excess contribution, I will include:
1. 1040X for 2019 with signature
2. 1040 for 2019 with new schedule 2 (additional taxes) with signature
3. 5329 forms without signature
I will not include the following forms that were originally filed for 2019 tax return since there is no changes on these forms:
1. schedule B for 2019 (interest and ordinary dividends)
2. form 8995 for 2019 (qualified business income deduction simplified computation)
I will withdraw the excess amount this year therefore have not received 1099r yet
Is there anything else that I missed?

Correct, except you do not need to sign the new 2019 Form 1040.
