Am filing for my father who is 95 and living in a long term care facility. He is on CT Medicaid, do I indicate that anywhere on his return?

No, but you can only have a deduction for medical expense over and above what they pay for his care facility. Medical expense can also include his cost for the medicaid insurance, if any.
