Where on your questionaire do I put form 1095-B, instead of form 1095-A?

A form 1095-B is not entered on a tax return.  You keep the form for your own records.

In the Health Insurance section of the program where asked if you had any of the less common plans listed, answer No since you have a Form 1095-B and had health care coverage for the whole year.  That will complete the Health Insurance section.
