Yes, you should have a Form 1095-B for you records.  Now, you only really need one for your records in case the IRS were to ever question down the road whether you had insurance in 2016, but its something that you would want to try to get so you have it just in case.

If you have your 2015 1095-B and the coverage provider was the same in 2016, then you can find the contact information for the coverage provider on Line 18 of Form 1095-B.  You should contact the provider about reissuing you a 2016 Form 1095-B.
