You may be on the wrong entry screen. Try this: click on the Federal Taxes tab, then Wages & Income, then scroll down to the income headings on the lower half of the page (you may need to click on "Skip to see all income" to display them).
Under the heading Less Common Income, select the last choice, Miscellaneous Income 1099-A, 1099-C. You'll be shown a new list of income types, and you'll select Cancellation of debt (Form 1099-C). Then follow the prompts.
TurboTax will not ask you for the date from box 1, but you'll see boxes 2 & 3 match your 1099-C form.
