Hi

For total of all payments and withholdings, should I add my city taxes I paid which was not on my W-2? Also, should I add my 2020 prepaid taxes too?

Yes and no.  Yes, add in your city taxes paid for 2019.  No, you are only adding in amounts for 2019 taxes paid here.
