for the Tax year 2024, I made excess contribution of $1000 to Roth IRA and had to initiate a return for excess contribution in Fidelity. Amount returned was $975. In Jan 2026, I got the Form 1099-R from Fidelity with Tax year as 2025. Box 2a is $0 and Box 4 & 14 is blank.
When I file the 2025 tax return, do I need to file form 5329?
Do I need to amend the 2024 tax return? If Yes, what needs to be filed?

No, you do not need to file Form 5329 because you withdrew your 2024 IRA overcontribution before the tax filing deadline in 2025. You also do not need to amend your 2024 tax return because you actually lost money on the contribution so there is no taxable income.
