I received a distribution (and 1099R) from my 401k under the COVID law in 2020. The law allows me to spread out the distribution over 3 years. I reported 1/3 for the 2020 tax year. I need to report 1/3 for the 2021 tax year.
However, my 401K administrator did not (and is not) providing me with a 1099R for 2021. I assume I need to enter in the 1/3 distribution on my own with no back up? Is this correct? Thank you.

Yes, the IRS is aware since you filed Form 8915-E last year declaring the one-third. This year, you will file 8915-F to report the amount for this year. It currently has an expected release date of 3/31.
