*In May, my employer switched from paying me in 1099-MISC to W-2. They never offered any health insurance options.
Can I deduct it as a self-employed business expense? For just the 4 months I received 1099-MISC income, or for the full year?

On the W-2 you received, is box 13 "statutory employee" checked.
If you are a statutory employee than you will continue running your business and you would add a W-2 to your return and make sure to check box 13.
