Zip Code for Foreign Dependent Care Provider

I have added all the information for a Canada-based (foreign) dependent care provider, but after choosing State: "Foreign or U.S. Territory," the form requires a ZIP code in the format ##### or #####-#### format. I cannot get past this screen. I've attempted to leave it blank, to use 00000, but I am stuck. I've placed the full address in the City field so that the IRS has all the information they need, but the app won't let me proceed.
