Can I open a HSA in January 2019 and still make a prior year contribution for 2018? We have been on a qualified HDHP since April 2018.

If yes, can I report the 2018 contribution without a tax form?  The contribution would be made directly from my bank account.  I appreciate the help.

Yes, you can make an HSA contributon prior to tax filing due date and report it on your 2018 return.  

And yes, you don't need a tax document (1099-SA) to report this on your tax return.

Type 'HSA' in the Search window, and click on 'Jump to hsa'.  

You will also be asked about your HDHP in this section.

If you open an HSA in January 2019, you can make a past year contribution for 2018, but only if you were covered by an HSA-eligible insurance plan starting on or before 12/1/18 (using the last month rule).  If you were only enrolled in an eligible plan beginning 1/1/19, you can’t make any 2018 contributions.
