Your HSA should l be code 12W and will flow automatically to form 8889 assuming your employer properly prepared the w-2. 12W includes both your HSA contributions and any match by the employer. they will transfer to line 9 of the 8889. do not re-enter on any other line such as line 2.
if you contributed to an HSA and there is no 12W on your w-2, contact your employer only it can fix the issue.

Figured out my problem. We actually have an FSA - NOT an HSA. So I was doing the wrong thing altogether. Thanks!
