Where do I put the Social Security Medicare Part B premiums on the Wisconsin form?

You don't. Those are entered in the federal section when you enter your SSA-1099 and TurboTax transfers these amounts to your state return.
Actually, no they do not ALWAYS transfer. Mine did not - I caught the issue when I compared it with a spreadsheet I did for WI form 1. (And last year I verified with WI DOR that this is in fact a valid entry to make - Line 11 Code 01).
In Forms Mode the trail is Form 1, Line 11 => "Addn, Subt. Stmt" Code 01 => Medical Care Insurance Worksheets => Workseet 2.
In Interview mode it is found in the INCOME section, under Medical Care Insurance. On the Medical Care Insurance there are three boxes on that interview form: Self-employed, Employee and Employee or Unemployed. For most retirees, it is that third box where you want to put that number.
