Why would my medicare premiums be added to line 1 total wages?

These are already included lin total SS earnings in box 5 of the 1099.

Why would my medicare premiums be added to line 1 total wages?

They wouldn't and shouldn't be added to your total wages if entered correctly.  I would revisit your SSA-1099 Form entries and see if perhaps you entered your medicare premiums in the correct spot.
