Why is it asking me for form 2441?

Form 2441 is for the Child & Dependent Care credit and deduction. Did you pay for child care expenses or expenses for someone like a parent?

Also, is there an amount in box 10 of your W-2? This is the amount of money set aside from your wages to fund your dependent care FSA. If this number is there (and if it is supposed to be there) then you will have to file the 2441.

If I get prompted - "Form 2441 - Provider SSN must be entered." but, I don't have an SSN as the provider refused to share their SSN. So how can I proceed with this?
