Why do the 1099-SA fields in the Turbotax form not match the information fields on the 1099-SA form I received from my HSA?

I cannot fill in the fields for your 1099-SA questions with the information provided on the 1099-SA form received from my HSA. What do you suggest?

To my knowledge, our data entry form DOES have the same fields as the IRS' 1099-SA.

Can you give us an example of what fields are different between your 1099-SA and our data entry screen for the 1099-SA in the HSA interview?
