Your HSA will be covered in three places.
Contributions made through your employer, either employer money or your money contributed by salary reduction, is recorded on your W-2.
If you contributed extra money out of pocket not by payroll deduction, that will be recorded on a form 5498 mailed to you by the HSA. However, because you can make contributions for the 2016 tax year as late as April 15 of 2017, the HSA bank probably won't send the form 5498 until May. In turbotax, there is a place where you are asked if you made contributions not included in payroll, you can enter them here even if you don't have the form 5498.
If you took distributions to pay for medical care you will receive a form 1099-SA from the HSA bank which must be mailed by January 31.
