The 2021 Form 2210 does not seem to match the official IRS form. In addition, the quarterly tax due and payment dates do not coincide with the 2021 filing year. Finally and most importantly, line 11 does not include all payments made and taxes carried over from 2020. Has anyone else had this problem and how was it resolved?

Look at line 26 on the 1040 and see if it matches the refund you wanted carried over and the estimated tax payments. If it doesn't match, manually enter the refund you wanted carried over to this year. Under "estimates and other taxes paid," go to "income taxes paid."
NO...the form 2210 is not ready yet, but supposed to be ready on the 17th of Feb.
A bunch of dates in the Temporary form, are still linked to 2020 dates and deadlines....thus it's not going to be useful until the coming weekend.
