The form asks for mortgage origination dates. The first one was in 2011, and I refinanced (same bank) in 2019, so there are two origination dates How do I handle this?

Enter the mortgage origination date that's in box 3 of your Form 1098.

You need to enter both forms one at a time ... enter the original mortgage first then follow the interview screens carefully to indicate the refinance.
