There would be nothing to report if you had no activity in your account as you have posted.
The only activity were the maintenance fee charges. I last made contributions in 2018 and completely forgot I even had the account until I started answering tax questions.
As stated by user Bsch4477, you would not need to fill out anything with regard to your HSA on your 2019 tax return if there were no contributions or distributions made during the year. Maintenance fee deductions do not count as a distribution and would not be reported on your return.
