On the medical expenses I'm not sure what to put in the Medical Premiums out of pocket section - I already put in my out-of-pocket expenses in the prior sections.

The Medical Premium out of pocket would be what you paid for your health care coverage.

It is worded that way so that a Taxpayer would not include amounts for coverage that an employer or someone else paid.
