My husband has an HSA. I have a question regarding total contributions.

We received a refund from his doctor (due to on over payment).  We had paid using our HSA.  We deposited the amount refunded back into our HSA.  That amount is showing as a part of the total contributions made in 2016 (form 5498-SA, box 2).  When asking if we made additional contributions (other than through employment) does that count as a contribution since we used our HSA to make the original payment?

It should not be counted as a contribution. It is a refund. You have already received the tax benefit when the funds were contributed the first time.
