All income is taxable unless specifically excluded. There is no exclusion for blood/plasma donations. If you are compensated for this, then the income earned is taxable.

Yes you should report all income on your tax return.

 

If you did not receive any Tax documents than you would report this as Other Income. Other income includes earnings other than wages or income from self-employment, retirement income, or investments, foreign income, and canceled debts. 

 

Other Income is generally taxable income that is considered to be not common income. It is reported on Line 8 of Schedule 1.
