My employer contributed $2k to my HSA. I did an addl $25/ch X 26 pay periods, but my $650 is not included in box 12 on my W. Can I enter the $650 I know to be right?

You should get a 'corrected' W-2 if yours is not correct.  Your employer has to give you and the IRS the corrected copy.  If you send your return in and your data doesn't match what is on file with the IRS, you may have undesirable issues.

Get the W-2 corrected.
