When deducting medical expenses, is this only in reference to out of pocket, or does it include the total of expense including what insurance covered?

Medical expenses

It includes only what you actually paid out of your pocket. What was billed does not matter, what insurance paid does not matter. All that matters is what actually came out of your pocket.
