"Medical insurance or HMO premiums you paid for you and your family (after tax premiums only)" how to interpret this sentence? Is that mean how much i spend with my credit card on seeing a doctor?

It is asking if you pay for the medical insurance premiums with after tax dollars. Generally if you are an employee and get health insurance through your employer they are paid for with pre-tax dollars (out of your paycheck).
If you are self-employed, generally you pay for the health insurance with after tax dollars.
