If I gave regularly to a charity, and the charity changed its name during the year, how do I report it? Do I use the old name, new name, or report both separately?

It needs to be listed in the IRS exempt organization database.  Use the name it is registered under.  If not registered, then it is not eligible for deductible donations, even though it may have a generally charitable purpose.
