A medical FSA (different from an HSA) doesn't go on your tax return. The only place you might enter it is if you list your medical expenses for deductions. If you list all your medical expenses, you will be asked later if you paid any of those expenses with a FSA or other tax free money, and that will reduce your deduction. But in other cases, you might not bother to enter expenses if you know they were covered by the FSA and so not deductible, so you won't see the FSA question.
An HSA is something different and must be reported if you had deposits or withdrawals.
