1040, and entered into Turbotax as follows:

tab

On the Personal
Income tab click Continue
·
Select I'll
choose what I work on
·
Scroll
down to Other Common Income
·
On the Income
from Form 1099-MISC line click Start
·
Enter the information from the 1099-MISC and click through the pages
·
When you see the page Does one of these uncommon situations apply? select I was in a research
study and this was payment, when the question Did you perform a service to earn the money This Payer paid you? select No
·
Complete the remaining screens
