If you made a QCD you still enter the 1099R for the full amount. Then a couple screens later it will ask if it was a QCD. Then the amount will be deducted from 1040 line 4b with the letters QCD by it.
How did you remove your RMD? You had to have the IRA account send the donation directly to the charity. You can not get the cash and send it yourself. I just did one and my IRA sent me a check but it was made out to the charity not me. I then mailed it to the charity.

I removed my RMD and never got credit for the $5,000,00 I gave to CHARITY .
This was for my 2021 TAXES. The RMD was taken in the full amount and Fidelity sent the checks (3) directly to the different Charities . I am trying to do an amended report and it does not come up on line. The charity amounts were $5,000.00. ?????
@Rose Cavanaugh look at form 1099-R from Fidelity ...whatever is listed as taxable income in box 2a needs to be reduced by $5,000 so that you get credit for the QCD.... Fidelity does not make that adjustment for you.
