I received back pay, a year after I left my previous job. The check I received was less than $500 and had all taxed deducted. The job was in another state as well. Do I need to report this?

Yes, you have to report it. They should have sent you a W-2. If you moved and didn't notify the former employer, they probably sent your W-2 to your old address. Call them and ask for another copy of the W-2, and give them your current address.
