There will not be any additional form for having "extra" money withheld from your paycheck. It will simply be included in the amount reported on your W-2. There is no special box on your W-2 for it either. Money withheld for federal is shown in box 2, and money withheld for the state will be in box 17.
It occurs to me that you said the state "I work in." Do you live in one state and work in a different state? Are you actually asking about the need to file a tax return in the state you work in? Please explain what you are trying to do.
Your other-state employer should already be withholding that state's taxes, unless you live in a reciprocal state. If you're adding an additional $50 to your withholding each pay period through your payroll department, it'll be reflected in your year-end W-2.
