1) Were you covered by a Deductible Health Plan for every month of 2025?
2) Did you have any other full medical insurance coverage under a plan that was not an HDHP (speciality insurance like for certain diseases, vision coverage and dental coverage don't count.
3) Were you covered by Medicare? There may be a 6-month lookback. You are not qualified to have an HDHP for months covered by Medicare.
4) Were you covered by a Flexible Savings Account? your ineligible to have an HSA.

@ejmilian the typical mistake is that you contributed $4300 via your paycheck and that shows as Code W on Box 12 on the W-2..... so far so good!
But then there is a question in the HSA section asking whether you contributed to your HSA; the question is asking about direct contributions and do NOT consider the $4300 in Box 12 of your W-2. Your answer should be zero!
