I just received Notice of Intent to seize (levy) property or rights to property. Notice CP504 Tax Year 2014

The letter states that they notified me before but that is not true. I didn't receive anything. As matter of face I received a small refund that year.

Call the IRS on Monday using the phone number on the notice you received and speak directly with an IRS agent.

I’m trying to file my 1099 miscellaneous for oil rights EQT does not have the address or the name of the property how am I supposed to file without it?
