I did not file an 8889 for my HSA. None of it was taxable (all qualified medical) distribution. Do I need to file an amended return?

Yes, you will likely want to file an amended return, but you cannot do so until the IRS has accepted and processed your return and you have received your refund.

There is a question in the 1099-SA interview that you need to answer when you prepare the amendment.  To revisit the interview:

• In the Federal return, choose Deduction & Credits
• Scroll down to Medical and Show more
• Start or Revisit HSA MSA Contributions and edit the appropriate 1099-SA
• Continue through the HSA MSA interview until you reach the screen "Did you spend your HSA money on medical expenses only?" select the radio button next to "Yes, I only used it for medical expenses."  

This should resolve your issue.
