I am over 55 with my employer paying HSA contribution totaling $340. Why does the form fill in the box with over $4,000?

did you through your employer make additional HSA contributions? all contributions by employer and employee through payroll deductions show up as employer contributions on the HSA tax form. they should be the same as line 12W on your w-2.
