Report each amount with its code.  W is your HSA contribution.

Enter amounts shown in box 12 of your W-2 only in box 12 of TurboTax's W-2 form, nowhere else in TurboTax.  TurboTax will automatically propagate the code-W amount to Form 8889 as an HSA contribution made through your employer.
