How do I show medical expenses for Arizona? ie just the total dollar amount or the amount from each doctor/prescriptions/glasses etc.

You should claim all of your medical expenses in the Schedule A, itemized deductions on  your federal return.

Your medical expenses will then carry over and populate the medical deduction field on your Arizona return.
