How do I fill out a 2018 form 1 personal property tax return?

It's for Maryland I printed out form 1 I'm stuck on the part that asks for a list of personal property from the last 7 years what I'm trying to understand is does it want my tax assessment from the county, my depreciation from my taxes or something else?
