Form 8889 line 2 is not showing my contributions

I contributed $7100 to my HSA fund.  However, line 2 on form 8889 is showing $0.  My contributions were not made thru payroll.  I contributed the $7100 after tax thru my personal checking account.  While entering my HSA info I answered that I personally contributed $7100.  Not sure why the form is showing $0

have you completed the 8889 section to answer the questions about insurance coverage?   we can't see what you did so don't really know what you didn't do to get the 8889 to populate.

There is a line that asks the question "any contributions you personally made but withdrew before April 18, 2022?"  Do I include the withdrawals I made for approved medical expenses or is this asking about non-medical approved withdrawals?
